Step 2:-

Start Data Loader by clicking it's icon and you will be presented with the Welcome Screen.

In this screen Click "Start New Session" link

Then you will be getting the Choose Source and Destination Database form.

Select MS SQL Server option button in Source Database Type Section and MySQL option button in Destination database type section and click "Next" Button.
